Services

The Neighborhood Nurse provides the following non-medical services:

• Provide at-home initial consultations

• Conduct care-planning assessments to identify obstacles to independent living 

• Assist in locating a senior living residence if needed

• Screen, arrange, and monitor in-home help and durable medical equipment

• Review medical issues, and make referrals where necessary

• Accompany clients to appointments with doctors,  etc.

• Conduct wellness checks and reporting for families

• Medication management
